SUMMARY OF INCIDENT:


Forino was eastbound on Rt 163 in T11 R4 at the T11 R4/Ashland town line when a moose entered the roadway. Forino, operating a 2006 Toyota Sienna van, was unable to avoid the moose and struck it, killing it. When the van struck the moose, the moose crashed through the windshield and into the passenger compartment of the van, narrowly missing Forino. Forino was not injured in the accident but did have some broken glass flushed from his eyes at the scene by Ashland Ambulance.
